Submitted by sponsor and co-sponsor, name of nominee, school or institution where they presently teach, school or institution address, statement of rationale for recommendation of the nominee, resume with educational and professional experience, service to professional and community organizations, area of ELA that the nominee has taught or is presently teaching, and any other supportive material that may be submitted for consideration.
